A comparison between Accu-Chek Performa and automatic dry chemistry Vitros 350 for oral glucose tolerance test in pregnancy

Abstract


Diabetes in pregnant women is harmful for both mother and baby. Therefore, the efficiency of screening test is important for the management of those patients. At Bangplee Hospital, Samutprakran, an automatic dry chemistry Vitros 350 (Vitros350) was used for glucose determination in routine laboratory, whereas the Accu-Chek Performa was used in a health station under the supervision of Bangplee Hospital. So, this study aimed to compare the accuracy of the oral glucose tolerance test (OGTT) in pregnant women using Vitros350 in parallel with Accu-Chek Performa. Thirty nine blood samples were collected from pregnant women and analyzed for OGTT. The samples were collected before the administration of glucose (time 0 h), after oral intaking glucose at 1, 2 and 3 h, respectively. All samples were analyzed its glucose concentrations by using Vitros350 and Accu-Chek Performa. The evaluation of data based on the criteria of National diabetic data group.  The results showed that 6 of 39 (15%) measured by Vitros350 and 9 of 39 (23%) by Accu-Chek Performa of pregnant women were classified as diabetes. The correlation of glucose at 0, 1, 2 and 3h were highly correlated between assay by Vitros350 and Accu-Chek with correlation coefficients (r) of 0.980, 0.986, 0.987, and 0.981, respectively. The glucose levels obtained from Accu-Chek Performa were demonstrated higher than that of Vitros350. Based on ISO 15179: 2003 criteria, the Accu-Chek Performa is considered to be acceptable for giving the diagnosis of pregnancy case. However, the paradoxical criteria between ISO 15179:2003 and the American Diabetes Association (ADA) are found. Therefore, the data analyzed based on ISO 15179:2003 criteria may not be consistent with ADA. In conclusion, non diabetic pregnant women gave no difference of OGTT either measured by Accu-Chek Performa or Vitros350 based on the ISO 15179: 2003 criteria. However, assay by Vitros350 is suggested to be done for the patient whom diagnosed as having diabetes by Accu-Chek.
