A Comparison of Hemoglobin Measurement between Hemoglobinometer(POCT) and Automated Blood Cell Analyzer in Hemodialysis Patient

It is essential to analyze the hemoglobin of the patients, who are falling in the end stage of chronic renal failure, before the hemodialysis treatment starts. The feasibility in conducting a less time consuming test prior to the hemodialysis will bring a great benefit in safety to the patient plus the reduction in service time. The aim of this work was to find the correlation value of hemoglobin in chronic renal failure patient measured by URIT – 12 (POCT) Meter compared with CBC result obtained by the automated blood cell analyzer. The sensitivity and specificity in diagnosing the anemic status of chronic renal failure patient using URIT – 12 Meter was also investigated. A total of 107 samples from 62 chronic renal failure (CRF) patients who received the hemodialysis treatment in Burirum hospital were studied. The paired-T test was used to analyze the correlation level of both methods. A positive correlation of hemoglobin value was obtained by both methods i.e., correlation = 0.830. However, the mean + 1SD of hemoglobin concentration from the automated blood cell analyzer was significantly lower than that measured by the URIT-12 method (9.46±2.00 and 11.32±2.54 respectively, p<0.05). The sensitivity of anemic diagnosis was 43.48% while the specificity was 100%. Although the hemoglobin value of CRF patient measured by POCT meter was higher than that obtained by the automated blood cell analyzer, the method provides a specific diagnosis for anemia. The results thus suggested that the POCT meter is suitable for use in hemodialysis unit.
